Prostat kanserindeki prognostik bulguların karşılaştırılması: Transrektal biyopsi ve radikal prostatektomi örnekleri

Özet
Prostat kanseri tanısı konarak radikal prostatektomi yapılan hastaların, prostat biyopsisi ve radikal prostatektomi spesimenlerindeki prognostik bulguları karşılaştırarak, prostat biyopsisinin prostat kanseri tanısı koyma ve Gleason skorunu belirleme açısından tutarlılığını değerlendirmeyi amaçladık. Trakya Üniversitesi Tıp Fakültesi Üroloji Bölümü'ne 15.03.2007–16.12.2016 tarihleri arasında başvuran, prostat biyopsisi uygulanan, prostat kanseri tanısı alan ve tedavi amacıyla radikal prostatektomi uygulanan hastalar arasından sistematik-tabakalı-rastgele örnekleme yapılarak 200 vaka çalışmaya dahil edildi. Prostatektomi ve biyopsi spesimenlerindeki Gleason skoru ve diğer prognostik bulgular değerlendirildi. Çalışmaya alınan 200 hastanın yaş ortalaması 61,5 (44-77), tanı öncesi PSA değerleri ortalaması 12,7 ng/ml (1-150) idi. Hastalara ortalama 12 (6-36) kadran prostat biyopsisi yapıldı. Biyopsi ve prostatektomi örneklerinde ortalama Gleason skoru, sırasıyla, 6,5 ve 6,8 olarak saptandı. Biyopsi yapılan 200 hastanın gleason skorları sırası ile 3 (%0,5), 4 (%1,5), 5 (%9), 6 (%48,5), 7 (%24), 8 (%10,5) 9 (%5), 10 (%1) iken radikal prostatektomi gleason skorları sırası ile 5 (%9,5), 6 (%31), 7 (%33,5), 8 (%9,5), 9 (%10,5), 10 (%1) olarak saptandı. 200 hastanın %50,5'sinin biyopsi ve prostatektomi örneklerinde Gleason skoru uyumlu iken, %49,5'inin uyumsuz olduğu gözlendi Sonuç olarak ince iğne biyopsi örneklerinin Gleason skorları ile prostatektomi örneklerinin Gleason skorları arasında anlamlı bir ilişki olduğu: ince iğne biyopsi örneklerinin, prostatektomi örneklerindeki Gleason skorunu saptamakta başarılı olduğu bulunmuştur. In this study; we aimed to compare the positive core numbers and Gleason scores of the patients who underwent radical prostatectomy after being diagnosed as prostate cancer to evaluate the consistency of Gleason scores of radical prostatectomy and prostate biopsy with respect to diagnose prostate cancer. In this study 200 patients with prostate cancer who were diagnosed with prostate biopsy and underwent radical prostatectomy between 15.03.2007 and 16.12.2016 were included. We compared Gleason scores and positive core numbers of radical prostatectomy specimens and prostate biopsy specimens to note the consistency of prostate biopsy. Mean age of 200 patients was 61.5 (44-77), mean pre-biopsy PSA value was 12.7 ng/ml (1-50). 12 (6-36) pieces of prostate were sampled. Mean Gleason score of radical prostatectomy specimens and prostate biopsy specimens were 6,5 and 6,8 respectively. Gleason scores of prostate biopsy specimens were 3 (%0.5) 4 (%1.5), 5 (%9), 6 (%48.5), 7 (%24), 8 (%10.5), 9 (%5) 10 (%1) respectively and Gleason scores of radical prostatectomy specimens were 5 (%9.5), 6 (%31), 7 (%33.5), 8 (%9.5), 9 (%10.5), 10 (%1), respectively. Gleason score of radical prostatectomy specimens and prostate biopsy specimens were consistent in %50.5 and inconsistent in %49.5 out of 200 patients. As a results, we have found that there is a significant correlation between the Gleason scores of prostate biopsy specimens and radical prostatectomy specimens. We have noted that Gleason score of prostate biopsy specimens are successful enough to predict the Gleason score of radical prostatectomy specimens.
