Sakrokoksigeal pilonidal sinüsün cerrahi tedavisinde karydakis flep ameliyatı ile primer kapamanın karşılaştırılması

Çalışmamızda, son zamanlarda Karydakis flep ameliyatının gündemde olması, nükslerin düşük olduğunun belirtilmesi üzerine, bu tekniği primer kapama tekniği ile karşılaştırmak amacı ile 2005 Aralık ve 2007 Nisan ayları arasında Trakya Üniversitesi Tıp Fakültesi Genel Cerrahi kliniğinde; pilonidal sinüs tanısı konmuş hastalar iki grup halinde ele alındı. I. gruba, eksizyon ve primer kapama yöntemi, II. gruba, Karydakis flep operasyonu uygulandı. Birinci grupta 33 hasta ve bunların 32 tanesi (%97) erkek, 1' i (%3), kadın, ikinci grupta 15 hasta ve bunların 14' ü (%93) erkek, 1' i (%6.7) kadın idi. Totalde ise 46' sı (%95.8) erkek, 2' si (%4.2) kadın idi. Tüm olgulara lokal anestezi uygulandı ve hepsine Penrose dren yerleştirildi, postoperatif birinci gün dren çekildi. Grup I olguları ortalama 6.8 ay, grup II olguları ortalama 6.2 ay takip edildi. Her iki grupta en sık rastlanılan komplikasyon, yara yerinde seroma ve akıntı oluşmasıdır. Takipleri süresince birinci grupta %3, ikinci grupta %6.7 oranında nüks gelişti.. Sonuç olarak; pilonidal sinüsün tedavisinde, uygulanması diğer metodlara göre daha kolay ve basit, iyileşme süresi daha kısa ve oluşan skar dokusunun çok az olması nedeniyle eksizyon ve primer kapamanın uygun olgularda seçilebilecek ameliyat şekli olabileceği kanaatindeyiz.
Patients diagnosed as sacrococcygeal pilonidal sinus are studied in two groups in our study to compare primary closure technique and Karydakis flap operation, which is actually and is told to have low recurrence rates, during the period between November 2005 and April 2007, in the department of general surgery, Trakya University Faculty of Medicine. In Group I: Excision and primary closure and in Group II: Karydakis Flap operation are performed. There are 48 patients (95.8% male, 4.2% female) in the study. In first group there were 33 patients [32 (97%) male, 1(3%) female]. There were 15 patients in the second group [14 (93%) male, 1 (7%) female]. All of the operations were performed under local anaesthesia and penrose drains were placed which were taken out in the first postoperative day. Mean follow-up time was approximately 6.8 months for first group and 6.2 months for the second group. During the follow-up period 3 % of the cases of the first group and 6.7% of the cases of the second group were relapsed. For both of the groups, the major complications were seroma and serous discharge. As a result; we concluded that excision and primary closure is an easier and simplier procedure according to other methods and can be safely performed for selected cases with expected early recovery and a better scar formation.
