Primer göğüs duvarı tümörlerinde sağ kalım ve prognozu etkileyen faktörler

| dc.description.abstract | Kemik ve yumuşak dokusundan köken alan primer göğüs duvarı tümörleri tüm malignitelerin de %1-1.5’ini oluşturur. 01.03.2007 ile 01.04.2016 tarihleri arasında primer göğüs duvarı tümörü nedeniyle opere edilen ortalama yaşları 50,7 (14-86) olan 10 erkek, 9 kadın olgu retrospektif olarak değerlendirildi. Semptomlar, 10 olguda (%52,6) şişlik, beş (%26,3) olguda ağrı, 3 olguda (%15,8) ağrı ve şişlik iken bir olgu asemptomatikti. Tümör 3 olguda (%15,7) kemik, 16 (%84,3) olguda yumuşak doku kaynaklıydı. Tüm olgulara makroskopik cerrahi sınır tümör negatif olacak şekilde geniş rezeksiyon uygulandı. Histopatolojik inceleme sonunda 6 (% 32) olguda benign ve 13 (%68) olguda malign göğüs duvarı neoplazmı olduğu raporlandı. Hastanede kalış süresi ortalama 11,4 (5-30) gündü. Dört olguda yelken göğüs, 1 olguda yara yeri enfeksiyonu ve bir olguda plörezi şeklinde toplam altı (%32) olguda postoperatif morbidite gelişti. Rezeksiyonda yapılan 10 (%52) olguda rekonstrüksiyon uygulandı. Bunların beşinde otolog greftler kullanılırken; 5’inde sentetik greftler kullanıldı. Malign olgularda ortalama sağ kalım 47 ay (3-107 ay), benign olgularda 57 ay (6-144 ay) olarak hesaplandı. Takipler esnasında 5 olguda (%26,3) nüks gelişti. Primer göğüs duvarı tümörlerinde lokal kontrol ve uzun süreli sağ kalımın geniş rezeksiyon ile sağlanabilineceği, ancak nüks açısından yakın takip edilmesi ve cerrahi sınırlarda şüphe olması durumunda cerrahi veya adjuvan tedavilerin tümör tipine ve olgunun fonksiyonel performansına göre yeniden değerlendirilmesinin fayda sağlayacağı görüşündeyiz. | en_US |

| dc.description.abstract | Primary chest wall tumors are originating from bone and soft tissue, consist 1-1.5% of all malignant tumors. We evaluated 10 male and 9 female patients with ages ranging from 14 to 86 years (mean 50,7), who had primary chest wall tumor and underwent surgery between 01.03.2007 and 01.04.2016, retrospectively. Symptoms were swelling in 10 (52,6%), thoracic pain in 5 (26,3%), both pain and swelling in 3 (15,8%) patients. There was no symptom in a patient. Tumors were originating from bone in 3 patients (15,7%) and soft tissue in 16 patients (84,3%). All tumors were widely resected with malignancy free margine. Histopathological outcomes were malign in 6 cases (32%) and benign in 13 cases (68%). There were six morbidities(32%) which is 4 flail chests, a wound infection and a pleural efussion. Ten patients underwent reconstruction: 5 autologous grafts, 5 synthetic grafts. The mean survival were 47 months (range: 3-107 months) in malignant cases and 57 month (range: 6-144 months) in bening cases. Recurrent tumor developed in 5 patients (26,3%). We concluded that wide resection of chest wall tumors is essential to adequate local control and survival. | en_US |
