| dc.description.abstract | Asitli siroz hastalarında ortaya çıkan spontan asit infeksiyonu, mortalitesi yüksek bir komplikasyondur. Çalışmamızda, spontan asit infeksiyonu sırasında, proinflamatuvar sitokinler olan interlökin-6 ve tümör nekrozis faktör-alfa'nın asit sıvısı ve serumda, nitrik oksit ve myeloperoksidaz ise asit sıvısındaki düzeylerini araştırmayı amaçladık.Çalışmaya alınan 40 hasta, steril asitli grup (25), kültür negatif nötrositik asit'li grup (10) ve spontan bakteriyel peritonit'li grup (5) olarak üç gruba ayrıldı. Hastalardan başvuru anında asit sıvısı ve serum örnekleri alınarak saklandı. Daha sonra tüm olguların asit sıvısı ve serumunda immunoradyometrik yöntemle interlökin-6 ve tümör nekrozis faktör-alfa, asit sıvılarında ELISA ile myeloperoksidaz ve enzimatik yöntemle nitrik oksit düzeyleri çalışılarak üç grup birbiriyle ayrı ayrı karşılaştırıldı. Steril asitli grubun, asit sıvısı ve serumdaki interlökin-6 ile asit sıvısındaki myeloperoksidaz düzeyleri, kültür negatif nötrositik asitli ve spontan bakteriyel peritonitli olguların interlökin-6 ve myeloperoksidaz düzeylerinden belirgin olarak düşük saptandı. Asit infeksiyonu tanısı açısından, asit ve serum IL-6 düzeyinin duyarlılığı %100 iken özgüllükleri sırasıyla %60 ve %80 idi. Myeloperoksidaz'ın özgüllüğü ise %85, duyarlılığı %100 olarak saptandı. Her üç grupta, asit sıvısındaki interlökin seviyesi serumdakinden anlamlı derecede yüksek, spontan bakteriyel peritonit grubunda, asit sıvısı ile serum interlökin-6 düzeyleri birbiriyle koreleydi. Asit sıvısı nitrik oksit seviyesi açısından karşılaştırıldığında hiçbir grup arasında fark olmadığı tesbit edildi. Tümör nekrozis faktör-alfa'nın asit sıvısındaki düzeyi, spontan bakteriyel peritonitli grupta, steril asitli gruba göre belirgin olarak yüksek iken, kültür negatif nötrositik asitli grup ile steril asitli ve spontan bakteriyel peritonitli gruplar arasında anlamlı bir fark yoktu. Karaciğer hastalığının şiddeti açısından mevcut parametreler karşılaştırıldığında, Child B ve C grupları, birbirine benzer sonuçlar gösterdi. Elde edilen bulgular, asitli kronik karaciğer parankim hastalarında, spontan asit infeksiyonu tanısında, asit sıvısı ve serumdaki interlökin-6 ile asitteki myeloperoksidaz düzeylerinin, konvansiyonel yöntemlere yardımcı olabileceğini, nitrik oksit ve tümör nekrozis faktör-alfa değerlerinin ise tanısal açıdan uygun olmadığını düşündürtmektedir. | en_US |
| dc.description.abstract | Spontaneous ascites infection which occurs in cirrhotic patients with ascites, is a high mortality rated complication. In our study, we aimed to investigate the levels of interleukin-6 and tumor necrosis factor-alpha in ascitic fluid and serum, nitric oxide and myeloperoxidase in ascitic fluid during spontaneous ascites infection.Forty patients included in the study, are divided into three groups as sterile ascitic fluid group (n=25), culture negative neutrocytic ascites group (n=10) and spontaneous bacterial peritonitis group (n=5). At admission, serum and ascites samples were taken and collected. Thereafter, interleukin-6 and tumor necrosis factor-alpha levels were measured in serum and ascitic fluid by immunoradiometric method, myeloperoxidase by ELISA and nitric oxide by enzymatic method in ascitic fluid and the three goups were compared in terms of these mediators. The sterile ascitic fluid group has significantly lower interleukin-6 serum and ascitic fluid levels and lower ascitic fluid myeloperoxidase levels than culture negative neutrocytic ascites and spontaneous bacterial peritonitis groups. For the diagnosis of ascites infection, ascitic fluid and serum IL-6 levels sensitivity was 100% whereas specificities were 60% and 80%, respectively. The sensitivity of myeloperoxidase was determined as 100% and specificity as 85%. In all three groups, ascitic fluid interleukin-6 levels were significantly higher than serum levels and also ascitic fluid and serum interleukin-6 levels were correlated in spontaneous bacterial peritonitis group. There was no difference between three groups for ascitic fluid nitric oxide levels. While tumor necrosis factor-alpha ascitic fluid level in spontaneous bacterial peritonitis group was significantly higher than steril ascitic fluid group, there was no difference between culture negative neutrocytic ascites group and steril ascitic fluid or spontaneous bacterial peritonitis group. When we compared the parameters according to the severity of liver disease, Child B and Chld C groups showed similar results. Our findings show that ascitic fluid and serum interleukin-6 and ascitic fluid myeloperoxidase measurements may help the conventinal methods for the diagnosis of spontaneous ascites infection. However, nitric oxide and tumor necrosis factor-alpha values seems not to have diagnostic value. | en_US |
