Özet
Ankilozan spondilit, ağrı ve ilerleyici sakatlığa yol açan kronik bir hastalıktır. Bu çalışmanın amacı ankilozan spondilitli hastalarda hasta eğitiminin hastalık parametreleri üzerine etkisini araştırmaktı. Bu çalışma modifiye New York kriterlerine göre kesin ankilozan spondilit tanısı almış toplam 41 hasta üzerinde yapıldı. Olgular rastgele olarak iki gruba ayrıldı. Birinci gruptaki hastalar, yataklı servise yatırılarak yapılandırılmış eğitim ve egzersiz programına alındı. Bu program 4-5 kişilik gruplara ayrı ayrı uygulandı. İkinci gruptaki hastalar rutin poliklinik takibine devam etti. Tüm hastalarda tedavi etkinliği Bath ankilozan spondilit fonksiyonel, Bath ankilozan spondilit hastalık aktivite, Bath ankilozan spondilit global, Bath ankilozan spondilit metroloji indeksleri, göğüs ekspansiyonu, solunum fonksiyon testi, short form-36, ?ankylosing spondylitis quality of life?, beck depresyon ölçeği ve laboratuvar parametreleri aracılığıyla incelendi. Hastalar başvuruda ve 3 ay sonunda değerlendirildi. Çalışmanın sonunda ankilozan spondilit eğitim grubunda Bath ankilozan spondilit fonksiyonel, Bath ankilozan spondilit hastalık aktivite ve Bath ankilozan spondilit global indekslerinde, göğüs ekspansiyonunda, short form-36 ve ?ankylosing spondylitis quality of life? yaşam kalitesi ölçeklerinde istatistiksel olarak anlamlı düzelme gözlenmiştir. Bath ankilozan spondilit metroloji indeksi, solunum fonksiyon testi ve Beck depresyon ölçeğinde anlamlı fark bulunmamıştır. Sonuç olarak, bu çalışma ile hasta eğitiminin ankilozan spondilit hastalarında fonksiyonel durum, hastalık aktivitesi, genel iyilik hali ve yaşam kalitesi üzerinde etkili bir yöntem olduğu sonucuna varılmıştır. Bir süre hastanede yatırılıp yapılan grup eğitimi, rutin poliklinik takibinden daha etkilidir.
The effectiveness of structured group education on ankylosing spondilitis patients Ankylosing spondylitis is a chronic disease causing pain and progressive disability. The aim of this study was to assess the effect of patient education program on disease parameters in patients with ankylosing spondylitis. This study was performed on 41 patients with the diagnosis of ankylosing spondylitis according to modified New York Criteria. The patients were randomly allocated into two groups. Structured education and exercise programs were applied to the first group patients who had been hospitalized for this aim. The program was performed seperately in subgroups consisting 4-5 patients each. The second group followed the routine outpatient department program. The effectiveness of the treatment was assessed by Bath ankylosing spondylitis functional, Bath ankylosing spondylitis disease activity, Bath ankylosing spondylitis global, Bath ankylosing spondylitis metrology indices, chest expansion, respiratory function tests, short form-36, ankylosing spondylitis quality of life scale, beck depression scale and laboratory parameters in all the patients. Patients were evaluated on admission and at the end of 3 months. Significant improvements in terms of Bath ankylosing spondylitis functional, Bath ankylosing spondylitis disease activity and Bath ankylosing spondylitis global indices, chest expansion, Short Form-36 and ankylosing spondylitis quality of life scale indices were observed in first group patients undertaking structured education, at the end of the study. No difference could be found in Bath ankylosing spondylitis metrology index, chest expansion, respiratory function tests and beck depression scale. As a result, it is concluded that patient education program is an effective method on function, disease activity, general well beeing and quality of life in patients with ankylosing spondylitis. The group education program in hospital is more effective than routine outpatient follow-up. Key words: Ankylosing spondylitis, patient education, quality of life
