Acknowledged statistical help and a better use of p-values: a proposal
    (Clinical & Exper Rheumatology, 2019) Dincses, E.; Guzelant, G.; Hatemi, G.; Sut, N.; Yazici, H.
Objective. The p-value is commonly misused. We hypothesised that a close cooperation with a statistician would go along with a more proper use of p-values. We considered a close cooperation present, when a statistician was a coauthor, or a formal statistical help was acknowledged in a study report. Methods. Randomised controlled trials published in 2015-16 in 4 widely read rheumatology journals were searched for a close cooperation with a statistician, the inclusion of effect sizes, confidence intervals, exact rather than relative p-values and the omission of p-values in tables depicting trial entry data. Results. There were only 28/133 (21%) articles in which a formal statistical help was acknowledged (Group I). The rest (Group II) gave no acknowledgement of a close cooperation. Reporting of effect sizes (96% vs. 71%) and exact p-values (88% vs. 69%) were more in Group I (p=0.01, and p=0.08, respectively). Conclusion. While a formal acknowledgement of a close cooperation was notably infrequent at 21%, this went along with improvement in some aspects of p-value reporting. If substantiated by further studies, we propose that a formally acknowledged statistical help should improve p-value reporting. Like all professionals, statisticians would like their name/office to be formally associated with their good work.

Changing the traditional p-value significance threshold to .005 does not decrease the number of statistically significant p-values in observational studies more than in randomised controlled trials

Differences in pain and fatigue perception among a group of rheumatoid arthritis patients in the United States and in Turkey who have similar disease activity and functional status
    (Clinical & Exper Rheumatology, 2010) Celik, S.; Fresko, I.; Sut, N.; Batumlu, N. M.; Yazici, H.; Yazici, Y.
Objectives. The concordance of patient reported outcomes in rheumatoid arthritis (RA) among different countries has not been studied in detail. We tried to determine the differences in pain and fatigue perception among a group of RA patients in the US and in Turkey who had similar disease activity and functional score in multidimensional health assessment questionnaire (MDHAQ FN). Methods. One hundred and thirty-seven RA patients from Turkey and 129 from the US were studied. An MDHAQ was obtained and a DAS-28 was calculated for each patient. Pain and fatigue perception was compared between the two groups after adjusting for age, sex, MDHAQ FN and DAS-28. Results. Turkish patients had less pain than their US counterparts when adjusted for MDHAQ FN, DAS-28, age and sex (3.56 (2.24) vs. 4.35 (2.23), p=0.005) whereas there was no difference infaligue between the two groups (3.85 (2.44) vs. 4.25 (2.45), p=0.194). When the patients with a DAS-28 score of above 5.1 and below 2.6 were compared in both groups, Turkish patients had again less pain albeit less in the high disease activity group. Conclusion. This study suggests that Turkish patients have less pain than the US patients when controlled for age, gender and MDHAQFN and DAS-28 scores. This is at odds to the conventional wisdom that pain perception is increased among the non-Western cultures.

Frequent oral ulceration during early disease may predict a severe disease course in males with Behcet's syndrome
    (Clinical & Exper Rheumatology, 2012) Hamuryudan, V.; Hatemi, G.; Sut, N.; Ugurlu, S.; Yurdakul, S.; Yazici, H.
Objective. The numbers and recurrence rates of mucocutaneous manifestations can be highly variable among patients with Behcet's syndrome (BS) but it is not known whether these differences influence the disease course at the long-term. Methods. We evaluated the outcome of 30 patients that made up the placebo arm of a 6 months controlled trial of thalidomide and looked at the relation between the frequencies of mucocutaneous manifestations during the trial and the development of major organ involvement necessitating immunosuppressives during the post-trial period. Results. Fifteen (50%) patients had received immunosuppresives for major organ involvement during the post-trial period. Patients receiving immunosuppressive treatment were significantly younger at the onset of BS compared to those who did not (24.5 +/- 5 vs. 29.7 +/- 3.8 SD years; p=0.003). The mean number of oral ulcers recorded throughout the trial was significantly higher among patients using immunosuppressives compared to those who did not (2.09 +/- 0.96 vs. 1.43 +/- 0.8; p=0.029). This significance disappeared when adjusted for age of onset of BS (p=0.16). ROC curve analysis showed that having 10 or more ulcers during 6 months has a sensitivity of 86.7% and a specificity of 53% for the subsequent necessity of immunosuppressive use. The same association was not true for genital ulcers, follicular lesions and erythema nodosum. Conclusion. These findings on a limited number of patients suggest that frequent occurrence of oral ulceration during the initial years of the disease may predict the development of major organ involvement in men with BS.

Pulmonary artery aneurysms in Behcet's syndrome: a review of the literature with emphasis on geographical differences
    (Clinical & Exper Rheumatology, 2015) Celik, S.; Yazici, Y.; Sut, N.; Yazici, H.
Objective. To investigate the frequency of Behcet's syndrome (BS) with pulmonary artery aneurysms (FAA) publications, the most lethal complication of BS, as reported from different countries and to provide a review of diagnostic techniques, treatment approaches and prognosis. Methods. Countries from each continent with a population of 4 million and over were chosen (n=128). A PubMed search for BS, FAA and the country name was conducted and 23 countries with BS and FAA were identified. The, full, texts of articles (n=91) were analysed for data including gender, age, accompanying Vascular findings, diagnostic techniques, treatment modalities and mortality rates. Results. A total of 207 (183 males, 24 females) patients with BS and FAA were reported in 91 articles originating from 23 countries. As expected there was a significant correlation (r=0.88, p<0.001) between the total number of articles about BS (n=4431) and those related to FAA and BS. In a simple linear regression analysis the number of BS and FAA articles from Japan was significantly below the identity line while in Turkey there was a propensity to publish more articles related to FAA than expected. One hundred and sixteen patients (56%) were treated with immunosuppressive therapy. Biologics were used only in 5 patients (2%). Of the 207 patients, 62 (30%) died. Conclusion. FAA is mostly reported as case reports from countries where BS is common. FAA might be uncommon in Japan. The prognosis of FAA could be getting better.
