Torakolomber bileşke lezyonlarında önden dekompresyon ve stabilizasyon
Abstract
Amaç: Omuriliğin önden basısına neden olan torakolomber bileşke lezyonlarında, önden dekompresyon ve Stabilizasyon ile ilgili deneyimlerimizin gözden geçirildi. Gereç ve yöntem: 1997-1999 yılları arasında torakolomber bileşke lezyonu nedeniyle ünden omurilik dekompresyonunu takiben Kaneda sistemi ile omurga stabilizasyonu yapılan 8 olgu incelendi ve literatürle karşılaştırılarak tartışıldı. Bulgular: Tüm olgularda tam omurilik dekompresyonu sağlandı. Kaneda cihazı ile ilgili ameliyat sırasında ve sonrasında herhangi bir komplikasyon görülmedi. Postoperatif akut miyokard enfarktüsü ve ampiyem nedeni ile 2 olgu kaybedildi. Sonuç: Çalışmamız omurga cisminden kaynaklanan torakolornber bileşke lejyonlarında ünden yaklaşımın dekompresyon ve omurga stabilizasyonu için güvenli ve etkili olduğunu ortaya koymaktadır. Purpose: In this study, the efficacy of anterior medulla spinalis decompression and stabilization of lesions causing anterior compression to the spinal cord at thoracolumbar junction was investigated. Methods: Data of a series of 8 patients with anterior thoracolumbar junction lesions treated with anterior decompression and stabilization to Kaneda system between 1997-1999 were examined and discussed with the literature Results: In all cases, total decompression of medulla spinalis was achieved. There were no complications related with Kaneda device intra- and postoperatively complications. Postoperatively, two patients died as a result of acute myocardial infarction and empyeme. Conclusion: Results have demonstrated that this surgical planning was reliable and effective in decompression of medulla spinalis and spine stabilization for the lesion resulting from vertebral body at thoracolumbar junction.
