Barsak mikrobiyal metabolizmasının koroner yavaş akım fenomenindeki rolü
Özet
Koroner yavaş akım fenomeni, tekrarlayan göğüs ağrılarından ani ölüme sebep
olabilecek süreçlerden sorumlu olabilmektedir. Koroner angiografik olarak tanısı konulmakta
olup bilinen tedavi yöntemi mevcut değildir. Bu nedenle bu fenomenin gelişmesini önlemek
ya da tedavi modalitesi bulmak önem arz etmektedir. Barsak mikrobiyal metabolizması tüm
organizmanın denge halinde çalışmasında büyük role sahiptir. Bu metabolizmanın işleyişinde
meydana gelecek bir problem sistemik olarak enflamasyonu başlatıp çeşitli hastalıkların
gelişimine neden olmaktadır. Kronik böbrek hastalığı, diyabetes mellitus, kardiyovasküler
hastalıklar başta olmak üzere çeşitli hastalıklardan sorumlu olduğu gösterilmiştir.
Daha önce literatürde yer almayan bir hipotez ise barsak mikrobiyal metabolizmasının
koroner yavaş akım fenomeni gelişiminde rolü olduğudur. Bu hipotezin doğruluğunu
araştırmak amacıyla bu çalışmada elektif endikasyonlar ile koroner angiografi yapılıp sonucu
normal koroner arter saptanan 40 olgu ve sonucu TIMI frame sayısı ile hesaplanıp koroner
yavaş akım fenomeni saptanan 40 olgu karşılaştırılmıştır. Bu olgulardan karaciğer fonksiyon
bozukluğu olanlar, kronik enflamatuvar hastalıkları olanlar, aktif enfeksiyonu olanlar,
antibiyotik kullananlar dışlanmıştır. Bu olguların cinsiyet dağılımları incelendiğinde kontrol
grubundaki olguların 15 ’i (%37,5)erkek olup yavaş akım grubundaki olguların 22 ’si (%55)
erkek olgulardan oluşmaktadır (p:0,1). Kontrol gruptaki olgulardan 10’u (%25)sigara
kullanmakta iken yavaş akım grubunda 16 (%55) olgu sigara içmektedir (p:0,1). Kontrol
grubunda ortalama yaş 57,2±10,2 iken yavaş akım grubunda 53,1±13,7 olarak izlendi (p:0,1).
Hipertansiyon izlenen olgular kontrol grubunda 22 (%55), yavaş akım grubunda 15 (%37,5)
olgu olarak izlendi (p:0,1).Yavaş akım grubu 7 (%17,5) ile kontrol grubu 6 (%15,0) arasında
diyabet görülme sıklığı için istatistiksel açıdan anlamlı bir fark bulunamamıştır (p:0,7).
Laboratuvar değerleri karşılaştırıldığında her iki grup arasında istatistiki olarak anlamlı fark
bulunamamıştır. Her iki grubun kan Trimetilamin N-oksit enzim düzeyleri karşılaştırıldığında
ise koroner yavaş akım fenomeni görülen olgularda bu enzim düzeylerinin anlamlı olarak
daha yüksek olduğu saptanmıştır (p:0,02). Yavaş akım grubunda Trimetilamin N-oksit
ortalama 62,03±18,89 ng/mliken kontrol grubunda Trimetilamin N-oksit ortalaması
52,87±17,28 ng/ml bulunmuştur. Trimetilamin N-oksit enzim düzeyine sigara, cinsiyet,
hipertansiyon ve diyabetes mellitusun etkisi regresyon analizi ile değerlendirilmiş ve
istatistiksel olarak anlamlı sonuçlar bulunamamıştır.
Sonuç olarak barsak mikrobiyal metabolizması koroner yavaş akım fenomeni
gelişiminde rol oynayabilir. Barsak mikrobiyal metabolizmasının normal fonksiyon
göstermesi ise koroner yavaş akım fenomeni gelişimini engelleyebilir. Bu alanda daha net ve
kesin sonuçlara yaklaşılması ve gösterilmesi için çok merkezli ve çok kapsamlı çalışmaların
gerçekleştirilmesi gerekmektedir. Coronary slow flow phenomenon may be responsible for processes that can lead to
sudden death from recurrent chest pain. Diagnosis is made coronary angiographically and
there is no known treatment method. Therefore, it is important to prevent the development of
this phenomenon or find treatment modality. Intestinal microbial metabolism plays a major
role in the balance of the whole organism. A problem in the functioning of this metabolism
initiates inflammation systemically and causes the development of various diseases. It has
been shown to be responsible for a variety of diseases, chronic kidney disease, diabetes
mellitus, cardiovascular diseases.
One hypothesis that was not previously included in the literature is that intestinal
microbial metabolism plays a role in the development of coronary slow flow phenomenon.
In order to investigate the accuracy of this hypothesis, in this study, 40 patients with elective
indications for coronary angiography were performed and a normal coronary artery was
detected, and 40 patients with a coronary slow flow phenomenon were compared. Of these
patients, those with impaired liver function, chronic inflammatory diseases, active infections,
and those using antibiotics were excluded. When the gender distribution of these patients is
examined, 15 (37.5%) of the patients in the control group are male and 22 (55%) of the
patients in the slow flow group are male (p:0.1). While 10 (25%) of the patients in the control
group smoke, 16 (55%) patients in the slow flow group smoke (p:02). While the mean age
was 57.2 ± 10.2 in the control group, it was 53.1 ± 13.7 in the slow flow group (p:0.1).
Patients with hypertension were observed as 22 (55%) in the control group and 15 (37.5%) in
the slow flow group (p:0.1). No statistically difference was found between the slow flow
group 7 (17.5%) and the control group 6 (15.0%) for the frequency of diabetes (p:0.7). When
the laboratory values were compared, no statistically significant difference was found
between the two groups. When blood Trimetilamin N-oxid enzyme levels of both groups were
compared, it was found that these enzyme levels were significantly higher in patients with
coronary slow flow phenomenon (p:0.02). While the mean Trimetilamin N-oxid in the slow
flow group was 62.03 ± 18.89ng/ml, the average of Trimetilamin N-oxid in the control group
was 52.87 ± 17.28ng/ml.
Consequently, intestinal microbial metabolism may play a role in the development of
coronary slow flow phenomenon. Normal functioning of intestinal microbial metabolism may
prevent the development of coronary slow flow phenomenon. Multicenter and comprehensive
studies should be carried out in order to approach and show clearer and definitive results in
this field.
