Streptomisin ototoksisitesinin transient evoked otoakustik emisyon ile monitörizasyonu

Abstract

Amaç: Streptomisin tedavisi sırasında oluşabilecek ototoksisite nedeniyle, transient evoked Otoakustik emisyonlarda (TEOAE) ve saf ton odyometride olası değişiklikleri belirlemek ve ototoksisite monitorizasyonunda her iki testi karşılaştırmak. Gereç ve Yöntem: Çalışmamızda, ortalama 1g/gün dozda streptomisin alan 38 olgunun toplam 73 kulağı, TEOAE ve saf ton odyometri ile streptomisin tedavisi öncesi, tedavi süresince ve tedavi bitiminden bir ay sonra izlendi. Ve her iki testin sonuçlan birbiri ile karşılaştırıldı. Bulgular: Olguların tedavi öncesi ve sonrası ölçümleri karşılaştırıldığında, saf ton odyogramlarında herhangi bir işitme kaybı bulgusu saptanmamıştır. TEOAE ölçümlerinde ise, reproduktibilite parametresinde anlamlı değişiklikler saptanmazken (p>0.05), emisyon amplitüdlerinde tedavi sonunda anlamlı düşüş saptanmıştır (p<0.05). Tedavi bitiminden bir ay sonra yapılan son kontrolde ise emisyon amplitüdlerinde tam olmayan bir artış (iyileşme) tespit edilmiştir. Sonuç: Streptomisin tedavisi sırasında saf ton odyoınetriye yansıyan herhangi bir işitme kaybı bulgusu olmaksızın meydana gelen TEOAE amplitüdlerindeki bu değişiklikler, muhtemelen streptomisinin erken ototoksik etkisi sonucu oluşan, iç kulağın geri dönüşümlü fonksiyonel değişikliklerini göstermektedir. Bu nedenle, streptomisin Ototoksisitesinin monitorizasyonunda, kalıcı işitme kaybı meydana gelmeden ototoksisite için yüksek riskli hastaların önceden saptanmasında, TEOAE saf ton odyometriden üstün olabilir.

Purpose: To determine the possible transient evoked otoacoustic emission (TEOAE) and pure tone audiometric changes due to streptomycin ototoxicity and to compare them in the monitorization of ototoxicity. Methods: Seventy-three ears of 38 subjects who received an average of 1g/day streptomycin were monitored. Pure tone audiograms and TEOAE's were performed before, during and at the end of the treatment and four weeks after the treatment. The results of these two tests were compared each other. Results: The pure tone audiograms showed no significant changes between the pre-treatment and post-treatment measures. In TEOAE measurements, there were no significant changes in the reproducibility, however, the amplitudes of the TEOAE's decreased significantly (p&lt;0.05). The reduction of TEOAE amplitudes tended to be partially reversible at the measurement four weeks after therapy. Conclusion: These partially changes in TEOAE amplitudes may reflect reversible functional changes in the inner ear before the irreversible changes, which could be detected by pure tone audiometry, occurred. TEOAE maybe a more valuable monitoring method than pure tone audiometry to identify those patients at highest risk for streptomycin ototoxicity before irreversible damage have occurred.
