In our study, we aimed to evaluate the beliefs about treatment and its effect on adherence to treatment in the Turkish population with inflammatory bowel disease. Methods: In the study, the Medication Adherence Report Scale and Beliefs about Medicines Scale scales were used to evaluate the treatment compliance and perception and beliefs about treatment. Characteristics that could affect treatment compliance were evaluated by statistical analysis. Results: A total of 253 patients, 167 with ulcerative colitis and 86 with Crohn's disease, were included in the study. The non-adherence rate to the treatment was found as 41.9% in ulcerative colitis and 24.4% in Crohn's disease (P =.006). Intentional (29.3% in ulcerative colitis and 16.3% in Crohn's disease [P =.031] and unintentional non-adherence to treatment (28.1% in ulcerative colitis, 16.3% in Crohn's disease [P =.037] were significantly higher in ulcerative colitis than in Crohn's disease. Female gender (odds ratio = 2.59, P =.005), low education level (odds ratio = 4.8, P =.015), distal involvement in ulcerative colitis (P =.014), and thoughts about the disease would last too soon in Crohn's disease (odds ratio = 4.17, P =.049) were risk factors for non-adherence to treatment. Conclusion: The negative perception of treatment in inflammatory bowel disease affects adherence to the treatment. Considering some social factors that affect adherence to the treatment and taking measures to enhance the adherence to treatment will increase the success of treatment.Öğe Cerebral sinus thrombosis in patients with inflammatory bowel disease: A case report(Baishideng Publishing Group Inc, 2005) Umit, Hasan; Asil, Talip; Celik, Yahya; Tezel, Ahmet; Dokmeci, Gulbin; Tuncbilek, Nermin; Utku, UfukInflammatory bowel disease (IBD) is an idiopathic inflammatory disease of the gastrointestinal tract. The pathophysiology of IBD is probably the result of the complex interaction of genetic susceptibility and environmental influences. There is a well-known risk of thrombosis in patients with IBD. We present the case of a 53-year-old man with ulcerative colitis, who spontaneously developed intracranial sinus thrombosis that was treated with low molecular weight heparin. Literature was searched to assess the frequency and characteristics of cerebral sinus thrombosis in IBD and the role of certain etiopathological factors in such thrombotic patients. All rights reserved.Öğe Dilate or wait for effective anti-inflammatory treatment for stenotic lesions associated with active inflammatory signs in Crohn's disease(B M J Publishing Group, 2011) Ustundag, Yucel; Tezel, Ahmet[Abstract Not Available]Öğe HBsAg Pozitif Hastalarda Hepatit Delta Virüsü Seroprevalansının Retrospektif Olarak Değerlendirilmesi(2022) Eryıldız, Canan; Özgün, Eray; Yuluğkural, Zerrin; Tezel, Ahmet; Şakru, NerminAmaç: Çalışmamızda, HBsAg pozitif hastalarda hepatit delta virüsü (HDV) seroprevalansının belirlenmesi ve anti-HDV saptanan hastalarda hepatit B serolojik belirteçleri ile çeşitli biyokimyasal parametrelerin incelenmesi amaçlandı. Yöntem: Hastanemize Ocak 2015-Aralık 2021 tarihleri arasında başvuran ve anti-HDV çalışılmak üzere laboratuvarımıza örneği gönderilen 1831 HBsAg pozitif hasta çalışmaya dâhil edildi. Hastaların aynı başvuru sırasında incelenen diğer hepatit B serolojik belirteçleri, HBV-DNA düzeyleri, anti-HCV ve anti-HDV test sonuçları ile karaciğer transaminazları, serum albümin ve bilirubin düzeyleri, trombosit sayısı ve INR değerleri de geriye dönük olarak hastane kayıtlarından incelendi. Bulgular: HBsAg pozitif hastaların %3.6’sında anti-HDV testi pozitif saptandı. Anti-HDV pozitiflik oranının erkek olgularda kadınlardan daha düşük olduğu belirlendi (p=0.009). Yıllara göre anti-HDV pozitiflik oranlarında istatistiksel olarak anlamlı farklılık görülmedi (p>0.05). Anti-HDV negatif olgularda HBV-DNA pozitifliğinin, anti-HDV pozitif hastalardan yüksek olduğu belirlendi (p=0.001). Anti-HDV negatif olgularda anti-HBe pozitiflik oranının, anti-HDV pozitif olgulardan yüksek olduğu saptandı (p=0.001). Delta antikoru saptanan hastalarda trombosit sayısı ve albümin değerlerinin antikor saptanmayan hastalara göre düşük olduğu görülürken, INR değerleri yüksek bulundu (p<0.01). Sonuç: HBsAg pozitif hastalarda %3.6 oranında saptanan anti-HDV pozitifliğinin ülkemizde özellikle batı bölgelerde yapılan çalışmalar ile uyumlu olduğu görülmektedir. Hepatit D enfeksiyonunun klinik seyri göz önüne alındığında, hepatit B enfeksiyonlu hastaların hepatit D açısından taranması ve izlemi bu hastalığın yönetimi açısından oldukça önem taşımaktadır.Öğe Helicobacter pylori ınfection and benign gastroduodenal diseases, data from the Trakya Region(2010) Ümit, Hasan Celalettin; Ünsal, Gülbin; Tezel, Ahmet; Soylu, Ali RızaAmaç: Helikobakter pilori (H. pilori) gastrik ve duodenal ülser, non-ülser dispepsi, atrofik gastrit, mide kanseri ve MALT lenfoması etiyolojisinde rol oynamaktadır. Çalışmamızda Trakya bölgesinde beniğn gastroduodenal hastalıklarda H. pilori infeksiyonu prevelansı araştırılmıştır. Gereç ve Yöntemler: Trakya Üniversitesi Tıp Fakültesi Endoskopi Ünitesine başvuran ve beniğn gastroduodenal hastalıklar saptanan 4714 hastada retrospektif olarak H. pilori infeksiyonu prevelansı araştırıldı. Bulgular: Hastaların %52.8'inde H. pilori pozitif saptandı. H. pilori bulber ülser hastalarında %65, gastrik ülserli hastalarda %60.2, gastritli hastalarda ise %48.4 oranında pozitif bulundu (p<0.001). Trakya bölgesi kökenli hastaların %52'sinde, Anadoludan göç eden hastaların %56.3'ünde, Avrupadan göç eden hastaların %48.7'sinde H. pilori pozitif bulundu (p=0.02). Helikobakter pilori kadınlarda %53.2, erkeklerde %52.2 oranında pozitifti (p=0.52). Üreaz pozitifliği 30-60 arasındaki hastalarda 30 yaşından küçükler ve 60 yaşından büyüklere göre daha fazlaydı. Sonuç: Trakya bölgesinde H. pilori infeksiyonu hem ülkemizin diğer bölgeleri ile hemde komşu avrupa ülkeleri ile benzer epidemiyolojik özellikler göstermektedir.Öğe Increased circulating platelet-neutrophil, platelet-monocyte complexes, and platelet activation in patients with ulcerative colitis(Wiley, 2006) Pamuk, Gulsum E.; Vural, Ozden; Turgut, Burhan; Dernir, Muzaffer; Umit, Hasan; Tezel, AhmetIt is reported that the incidence of thromboembolism is increased in ulcerative colitis (UC), and hypercoagulability persists even when patients are in remission. We evaluated the association of inflammatory response parameters with UC activity, and activation parameters of the platelets, endothelium, and the coagulation system in UC. Eighteen UC patients and 19 healthy subjects were included in the study. The patients' clinical features were recorded down; whole blood counts and acute phase parameters were evaluated. UC patients were divided into two as active (9 patients) and inactive (9 patients) according to combined clinical activity index (CAI) and endoscopic activity index (EAI) scores. In all subjects, platelet CD62P expression, platelet-monocyte complexes (PMC), platelet-neutrophil complexes (PNC), and platelet microparticles (PMP) were determined by flow cytometry. E-selectin, thrombin-antithrombin complex (TAT) levels in plasma, and sCD40L levels in serum were determined by ELISA. In both active and inactive UC patients, platelet CD62P expression, the percentages of PMC, and PNC were significantly higher than those in the control group (P < 0.01). PMP level was higher in the control group than in inactive UC patients (P = 0.001). sCD40L level was significantly higher in active UC group than in the control group (P < 0.01). EAI score correlated significantly with PMP (r = 0.5, P = 0.04) and sCD40L (r = 0.48, P = 0.044); CAI score had a negative correlation (r = -0.68, P = 0.002) with sE-selectin level. In addition to increased CD62P expression and sCD40L, increased formation of PMC and PNC suggests a role for platelet-leukocyte complex formation together with platelet activation in thromboembolic events observed in UC. (c) 2006 Wiley-Liss, Inc.Öğe İnflamatuvar Barsak Hastalıklarında Osteoporoz Sıklığı(2019) Dilekci, Erdal; Tezel, Ahmet; Can, Güray; Demirkol, Mehmet Emin; Dilekçi, Esra Nur Ademoğlu; Gürler, Müjgan; Erkuş, Edipİnflamatuvar barsak hastalıkları, ağırlıklı olarak gastrointestinal sistemi tutan sistemik kronik inflamatuvar bir hastalık grubudur. İnflamatuvar barsak hastalıklarında ekstraintestinal bulgular sıklıkla birlikte bulunmaktadır. Bunlar içinde metabolik kemik hastalıkları azımsanmayacak ölçüde sıktır. Çalışmamızda kendi inflamatuvar barsak hastalıkları kohortumuzda osteoporoz ve osteopeni sıklığını ortaya koymayı amaçladık. Bu çalışmaya 71 ülseratif kolit, 44 Crohn hastasının kemik mineral dansite verileri retrospektif olarak değerlendirildi. İnflamatuvar barsak hastalığında tanı, tutulum paterni, hastalık davranışı, cinsiyet ve kemik mineral dansite lokalizasyonlarına göre kemik dansiteleri, T ve Z skorları karşılaştırıldı. Ülseratif kolit hastalarının %53,5’inde, Crohn hastalarının %50’sinde osteopeni saptandı. Ülseratif kolit ile Crohn hastalığı arasında anlamlı fark yoktu. Ülseratif kolit hastalarının %22,5’inde, Crohn hastalarının %25’inde osteoporoz saptandı. Ülseratif kolit ile Crohn hastalığı arasında anlamlı fark yoktu. Crohn hastalığında osteopeni en sık femur boynu ve wards üçgeninde (%47,7 ve %47,7), ülseratif kolit hastalarında osteopeni en sık wards üçgeninde (%52,1) saptandı. Ülseratif kolit ve Crohn hastalığında osteoporoz en sık lomber bölgede (sırasıyla %15,5 ve %18,2) tespit edildi. Ülseratif kolit ve Crohn hastalığı arasında osteoporoz ve osteopeni açısından anlamlı fark izlenmedi. Ülseratif kolit ve Crohn hastalığında alttip, davranış, cinsiyet ve kemik mineral dansite lokalizasyon açısından karşılaştırıldığında anlamlı fark saptanmadı. İnflamatuvar barsak hastalığında osteoporoz ve osteopeni sıklığı artmıştır. Bulgularımızı doğrulamak için daha büyük örneklem büyüklüğüyle ve iyi dizayn edilmiş prospektif çalışmalara ihtiyaç vardır.Öğe Inflammatory Bowel Disease and Thrombosis(Galenos Yayincilik, 2012) Tezel, Ahmet; Demir, MuzafferInflammatory Bowel Disease (IBD) is a group of chronic and relapsing inflammatory disorders of the gastrointestinal system. In these cases, findings are detected in extraintestinal systems also. There is a tendency for thrombotic events in IBD, as in the other inflammatory processes. The pathogenesis of this thrombotic tendency is multidimensional, including lack of natural anticoagulants, prothrombotic media induced via the inflammatory process, long-term sedentary life style, steroid use, surgery, and catheter placement. The aim of this review was to highlight the positive relationship between IBD and thrombotic events, and the proper treatment of at-risk patients.Öğe Investigation of IL23R, JAK2, and STAT3 gene polymorphisms and gene-gene interactions in Crohn's disease and ulcerative colitis in a Turkish population(Aves, 2016) Can, Guray; Tezel, Ahmet; Gurkan, Hakan; Tozkir, Hilmi; Unsal, Gulbin; Soylu, Ali Riza; Umit, Hasan CelalettinBackground/Aims: Inflammatory bowel diseases are chronic, relapsing, inflammatory conditions. They have a genetic backround resulting in patient susceptibility. The aim of our study is to investigate the involvement of IL23R, JAK2, and STAT3 polymorphisms in inflammatory bowel diseases in a Turkish population. Materials and Methods: Polymorphisms in IL23R (rs11209026), JAK2 (rs10758669), and STAT3 (rs3816769, rs2293152, rs744166, rs957970, rs8074524) were genotyped in 69 Crohn's disease patients, 157 ulcerative colitis patients, and 89 healthy controls. Results: The presence of (C) in rs10758669, (T) and (TT) in rs957970, and (TT) in rs744166 were found to increase the susceptibility to Crohn's disease (p=0.049, p=0.016, p=0.010, p=0.035, respectively), while rs2293152 (GC), rs744166 (CT), and rs957970 (CT) provide protection against Crohn's disease (p=0.007, p=0.043, p=0.043, respectively). While rs2293152 (GC) was protective, rs2293152 (CC) increased the susceptibility to ulcerative colitis (p=0.009, p=0.001). All the polymorphisms were associated with age-at-diagnosis, except rs11209026. Furthermore, rs2293152 was associated with an extension in ulcerative colitis, while rs10758669, rs3816769, rs744166, rs2293152, and rs957970 were associated with the subphenotype in Crohn's disease. The presence of rs10758669 (AC) was protective against perianal Crohn's disease (p=0.016). Additionally, rs10758669 and rs2293152 in Crohn's disease and rs8074524, rs3816769, and rs10758669 in ulcerative colitis were associated with the requirement of immunsuppression. Finally, rs8074524 and rs10758669 in Crohn's disease and rs11209026 in ulcerative colitis were associated with disease-related operation. Conclusion: This is the first study of the single marker association of IL23R, JAK2, and STAT3 polymorphisms with ulcerative colitis and Crohn's disease in a Turkish population. It was demonstrated that these polymorphisms may be effective in the etiology of inflammatory bowel disease in this Turkish population.Öğe Nötrositik asitli siroz hastalarında serum ve asit sıvısı TNF-alfa, interlökin-6, nitrik oksit ve myeloperoksidaz seviyelerinin, asit enfeksiyonu açısından tanısal ve prediktif değeri(2020) Harmandar, Ferda Akbay; Harmandar, Orbay; Tezel, AhmetGiriş ve Amaç: Asitli siroz hastalarında ortaya çıkan spontan asit enfek-siyonu, mortalitesi yüksek bir komplikasyondur. Çalışmamızda, spontan asit enfeksiyonu sırasında, proinflamatuvar sitokinler olan interlökin-6 ve tümör nekrozis faktör-alfanın asit sıvısı ve serumda, nitrik oksit ve myeloperoksidazın ise asit sıvısındaki düzeylerine bakıp, bu sonuçların asit enfeksiyonu açısından tanısal ve prediktif değeri olup olmadığını araştırdık. Gereç ve Yöntem: Çalışmaya alınan 40 hasta, steril asitli grup, kültür negatif nötrositik asitli grup ve spontan bakteriyel perito-nitli grup olarak ayrıldı. Hastalardan başvuru anında asit sıvısı ve serum örnekleri alınarak saklandı. Daha sonra tüm olguların asit sıvısı ve se-rumunda immünoradyometrik yöntemle interlökin-6 ve tümör nekro-zis faktör-alfa, asit sıvılarında ELISA ile myeloperoksidaz ve enzimatik yöntemle nitrik oksit düzeyleri çalışılarak üç grup birbiriyle ayrı ayrı kar-şılaştırıldı. Bulgular: Olguların asit sıvısı ve serum interlökin-6 düzeyi ortalaması kültür negatif nötrositik asit ve spontan bakteriyel peritonit grubunda steril asit grubundan anlamlı derecede yüksek bulunmuştur. Asit sıvısı tümör nekrozis faktör-alfa düzeyi ortalaması spontan bakte-riyel peritonit grubunda steril asit grubuna kıyasla anlamlı derecede yüksek saptanırken, serum tümör nekrozis faktör-alfa düzeyi ortalama-sında her üç grup arasında istatistiksel anlamlı fark saptanmamıştır. Asit sıvısı myeloperoksidaz düzeyi ortalaması kültür negatif nötrositik asit ve spontan bakteriyel peritonit grubunda steril asit grubundan anlamlı derecede yüksek bulunmuştur. Asit sıvısı nitrik oksit düzeyi ortalaması arasında ise her üç grup arasında istatistiksel anlamlı fark saptanma-mıştır. Sonuç: Elde edilen bulgular, asitli siroz hastalarında, spontan asit enfeksiyonu tanısında, asit sıvısı ve serumdaki interlökin-6 ile asit-teki myeloperoksidaz düzeylerinin, konvansiyonel yöntemlere yardımcı olabileceğini, nitrik oksit ve tümör nekrozis faktör-alfa değerlerinin ise tanısal açıdan uygun olmadığını düşündürmektedir.Öğe The Relationship Between Virulence Factors of Helicobacter pylori and Severity of Gastritis in Infected Patients(Springer, 2009) Umit, Hasan; Tezel, Ahmet; Bukavaz, Sebnem; Unsal, Gulbin; Otkun, Muserref; Soylu, Ali Riza; Tucer, DilekThe outcome of Helicobacter pylori infection has been related to specific virulence-associated bacterial genotypes. The best known genotypic virulence factors of H. pylori are cytotoxin-associated gene A (cagA) and vacuolating cytotoxin gene A (vacA). The objective of this study was to assess the relationship between H. pylori cagA and vacA status and histopathological findings. Esophagogastrodoedonoscopy was performed in 80 dyspeptic patients. Antrum and corpus biopsies were obtained for isolation of H. pylori and for histopathological assessment. The polymerase chain reaction was used to detect cagA and vacA genes of H. pylori using specific primers. Biopsy samples were stained with hematoxylin and eosin, and histopathological findings were graded using the updated Sydney system. H. pylori from 57 of the 80 patients was incubated. Of the 57 patients, 44 were cagA positive. In the corpus biopsy specimens there was a significant relationship between the density of H. pylori colonization (P = 0.02) and chronic inflammation (P = 0.02) and cagA-positive genotypes. In the antrum specimens there was a significant relationship between cagA positivity and neutrophil activity (P = 0.003) and glandular atrophy (P = 0.002), but not with H. pylori density, chronic inflammation, and intestinal metaplasia. The odds ratio of cagA-positive vs. cagA-negative strains for the presence of glandular atrophy, irrespective of grading and of gastric localization, was 4.62 (95% CI, 1.18-18.08, P = 0.041). No significant relationships were observed between vacA s1 and s2 genotypes and histopathological parameters. Corpus neutrophil infiltration was found to be more severe in the m1 group than in the m2 group (P = 0.004). Other histopathological features showed no difference between m1 and m2 genotypes. In conclusion H. pylori strains showing cagA positivity are associated with more severe gastritis in some histological features but virulence factors of H. pylori do not appear to determine the overall pattern of gastritis.Öğe REVERSIBLE HEPATOCEREBRAL DEGENERATION-LIKE SYNDROME DUE TO PORTOVENOUS SHUNTS(Literatura Medica, 2013) Guler, Sibel; Utku, Ufuk; Tezel, Ahmet; Unlu, ErcumentAtaxia and tremor are rare manifestations of hepatocerebral degeneration due to portovenous shunts. Ammonia is a neurotoxin that plays a significant role in the pathogenesis of hepatic encephalopathy. A 58-year old male patient was assessed with the complaints of gait disturbance, hand tremor, and impairment of speech. His neurological examination revealed dysarthric speech and ataxic gait. Bilateral kinetic tremor was noted, and deep tendon reflexes of the patient were hyperactive. Serum ammonia level was found to be 156.9 mu g/dL. Cranial magnetic resonance (MR) imaging revealed increased signal intensity in bilateral globus pallidus on T1 -weighted axial sections, and bilateral prominent hyperintense lesions in the middle cerebellar peduncles on T2-weighted axial sections. On his abdominal MR portography, multiple portohepatic venous collaterals were noted in the right and left lobes of liver parenchyma in 2D FIESTA axial MR sections. To our knowledge, we reported the first case of acquired hepatocerebral degeneration presenting with cerebral symptoms without any hepatic findings in which clinical improvement was noted, and hyperammonemia disappeared following medical treatment.Öğe Turkish IBD Organization's Position Statement on Inflammatory Bowel Disease Management Recommendations During COVID-19 Pandemic(Aves, 2021) Toruner, Murat; Kalkan, Ismail Hakki; Akyuz, Filiz; Tezel, Ahmet; Celik, Aykut FerhatThe COVID-19 pandemic, caused by the novel severe acute respiratory syndrome coronavirus 2, has resulted in high mortality and morbidity worldwide and is still a growing problem. Inflammatory bowel disease (IBD) is a chronic inflammatory disease for which a substantial number of patients are treated with immunosuppressive medications, either occasionally or long-term. Despite the accumulating evidence, there is still a lack of knowledge about the impact of COVID-19 on IBD patients, especially those who are under immunosuppressive treatment. Moreover, following the emergence of several COVID vaccines, there are concerns regarding vaccine effectiveness and possible side effects in such patients. Lots of genes have been identified resulting in susceptibility to inflammatory bowel disease. Any polymorphism leading to functional modifications in tyrosine kinase-2 may precipitate excessive immune response in the intestinal mucosa. The aim of our study is to investigate the involvement of tyrosine kinase-2 polymorphisms in the patients with inflammatory bowel disease in Turkish population. Methods: Four single nucleotide polymorphisms in tyrosine kinase-2 (rs280523, rs2304256, rs280519 and rs280496) were genotyped in 60 Crohn's disease, 151 ulcerative colitis patients and 89 unrelated healthy controls. These polymorphisms were detected by real-time polymerase chain reaction. Results: The presence of genotype (CC) in rs2304256 and (AA) in rs280519 were found to increase the susceptibility to ulcerative colitis (P = 0.024, 0.025, respectively). rs2304256 (CA) and rs280519 (AG) have provided protection against ulcerative colitis (P = 0.021, 0.012, respectively). rs280519 (AG) was protective against Crohn's disease (P = 0.045). rs2304256 (CC) increased the susceptibility to inflammatory Crohn's disease (P = 0.014). The presence of rs2304256 (A) increased the susceptibility to perianal Crohn's disease (P = 0.03). Both rs280519 and rs2304256 polymorphisms were associated with the requirement of corticosteroid and immunosuppressive therapy in ulcerative colitis. Conclusion: This study is the first demonstration of the single marker association of tyrosine kinase-2 polymorphisms with ulcerative colitis and Crohn's disease in Turkish population. They may be effective in the etiology of inflammatory bowel disease in our population. Disparity between our study and others may be related to ethnic differences. (C) 2015 Elsevier Masson SAS.
