Acute coronary syndrome resulting from an LMCA occlusion is associated with a significant morbidity and mortality rate, if it is managed with fibrinolysis. Electrocardiography can predict LMCA occlusion in patients with acute coronary syndrome. We report a 52-year-old male who presented with acute coronary syndrome and ostial LMCA occlusion. Its occurrence after coronary angiography is far less common. A possible mechanism of this complication may be contrast penetration of the blood-brain barrier with direct neurotoxicity to the occipital cortex.
