Öğe Adequacy of surgical margins, re-excision, and evaluation of factors associated with recurrence: a retrospective study of 769 basal cell carcinomas(Elsevier Science Inc, 2023) Urun, Yildiz Gursel; Can, Nuray; Bagis, Merve; Solak, Sezgi Sarikaya; Urun, MustafaBackground: Achieving adequate surgical margins and preventing recurrence are important in the treatment of basal cell carcinoma (BCC). Objectives: The objectives of this study were to evaluate the adequacy of surgical margins and the re-excision rates in patients with primary BCC who underwent standard surgical treatment using our proposed algorithm and to define the risk factors in patients with recurrent BCC. Methods: The medical records of patients who were histopathologically diagnosed with BCC were reviewed. An algorithm created based on previous literature was used to determine the distribution of optimal surgical margins adequacy and re-excision rates. Results: Statistically significant differences were observed between the cases with and without recurrence in age at diagnosis (p = 0.004), tumor size (p = 0.023), tumor location in the H zone of the face (p = 0.005), and aggressive histopathological subtype (p = 0.000). When the tumors were evaluated for adequacy of deep and lateral surgical margins and re-excision rates, higher rates of adequate excision (457 cases, 68.0%) and re-excision (43 cases, 33.9%) were noted for tumors in the H or M zone. Study limitations: Inadequate follow-up of newly diagnosed patients in terms of recurrence and metastasis and the retrospective application of our proposed algorithm are the limitations of the present study. Conclusions: Our results showed that if BCC was detected at an early age and at an early stage, recurrence was lower. Materials and Methods: A cross-sectional, electronic questionnaire was mailed to dermatologsts who work as an assistant professor or faculty member of dermatology in a university or training and research hospital in Turkey. The questions were prepared to evaluate views of young academic dermatologsts on academic dermatology. Results: A total of 37 assistant professors and faculty members of dermatology responded to the survey. The top three reasons for pursuing an academic career and the most satisfying and pleasing aspects of academic life were the opportunity to teach, do research, and deal with complex diseases. Compensation was the least frequently (5.4%) chosen reason to pursue an academic life. Insufficient time (70.3%) was stated to be the most challenging aspect of research activities. The most cited issues respondents stated that they needed support or education were data analysis (78.4%) and training abroad (67.6%). Conclusion: To our knowledge, this is the first study to evaluate academic dermatology in Turkey. We believe that the present study results provide insight into academic dermatology and might help increase the awareness of the dermatology residents who pursue an academic career.Öğe A case of extragenital linear lichen sclerosus along the lines of Blaschko responding to methotrexate(Dermatovenerological Soc Slovenia, 2020) Urun, Mustafa; Urun, Yildiz Gursel; Solak, Sezgi SarikayaLichen sclerosus, a rare, chronic, inflammatory, mucocutaneous disorder of the genital and extragenital skin, is usually asymptomatic and affects both sexes. The exact cause of lichen sclerosus is unknown. Extragenital lichen sclerosus may be localized or disseminated. Linear extragenital lichen sclerosus following the lines of Blaschko is an exceptionally rare form. A 66-year-old female patient presented with a sclerotic plaque extending from the dorsum of the right hand toward the elbow. The lesion first appeared on the right hand but spread toward the elbow within 1 year. The histological findings were consistent with a diagnosis of lichen sclerosus. We therefore diagnosed extragenital linear lichen sclerosus and achieved a good response with methotrexate.Öğe Cephalosporin-induced linear IgA dermatosis in a child: Case report and literature review(Wiley, 2019) Solak, Sezgi Sarikaya; Ficicioglu, SezinLinear IgA dermatosis (LAD) is a rare, subepidermal blistering disease with mucocutaneous involvement. It may be idiopathic or drug induced. We describe a 4-year-old girl who presented with a vesiculobullous eruption after she had been treated with cefixime for urinary tract infection. A diagnosis of drug-induced LAD was made based on clinical, histopathological, and immunofluorescence findings. Naranjo adverse drug reaction algorithm was used to assess imputability resulting with a probable association. In literature, cephalosporin antibiotics are rarely reported in association with LAD. To our knowledge, this is the first case of a cefixime-induced LAD among adults and children.Öğe Evaluating the differences among dermatologists' approaches to abnormal laboratory results of patients using oral isotretinoin treatment for acne(Wolters Kluwer Medknow Publications, 2023) Solak, Sezgi Sarikaya; Yelgen, Hande; Boga, ImranBackground: Oral isotretinoin is one of the most frequently used treatment options in moderate and severe acne. Abnormal laboratory results may occur during the treatment and there may be differences in approach to these abnormal laboratory results among dermatologists. Aim: In this study, we aimed to retrospectively evaluate the differences in approach to abnormal laboratory results and treatment modifications of dermatologists during oral isotretinoin treatment. Materials and Methods: Data of 207 patients who had oral isotretinoin treatment for acne between January 2013 and October 2020 were included in this study. Baseline and follow-up laboratory results were reviewed. All treatment modifications were noted and evaluated with relevant literature. Results: Among 207 patients, 28 (13.5%) had treatment modifications due to the abnormal laboratory results, and all of them were due to elevation of lipid and liver enzyme levels. The dose was reduced in 24 (11.6%) patients and the treatment was discontinued in 4 (1.9%) patients. Treatment modification was not compulsory in the vast majority of patients (26 of 28) according to the relevant literature. Conclusion: The results of the present study showed that unnecessary treatment modifications due to the abnormal laboratory results can be made by dermatologists during oral isotretinoin treatment for acne. Educational programs for dermatologists and more detailed guidelines may prevent these unnecessary treatment modifications.Öğe Evaluation of the quality, reliability, and popularity of Turkish YouTube videos on acne treatment(Galenos Publ House, 2022) Kaya, Ozge; Solak, Sezgi SarikayaBackground and Design: Social media is extremely popular for obtaining information on dermatological diseases and their treatments. The present study aimed to evaluate Turkish YouTube videos on acne treatment and compare them in terms of quality, reliability, and popularity based on the upload source.Materials and Methods: The first 120 Turkish YouTube videos on acne treatments were reviewed. The number of views, likes, dislikes, comments, video age, and video length were recorded. The quality and reliability of the videos were evaluated with the video power index (VPI). Then, the findings were compared based on the upload source.Results: A total of 104 videos uploaded by 52 physicians and 52 non-physicians (female: male ratio: 3.3:1) were assessed in this study. The DISCERN Score and Global Quality Score (GQS) were higher in the physicians' group (n=52, 50%) (p<0.001). The number of views, likes, dislikes, comments and VPI were significantly higher in the non-physicians group. The time elapsed since the upload day was longer in the videos uploaded by physicians, and the video duration was longer in the videos uploaded by non-physicians (p<0.001). The DISCERN and GQS in videos uploaded by dermatologists (n=30, 54%) were higher (p<0.001) than in those uploaded by non-dermatologists (n=22, 46%). No statistically significant difference was found in other findings.Conclusion: This study's findings confirmed that Turkish YouTube videos on acne treatment uploaded by doctors, especially dermatologists, were of higher quality and reliable, albeit with lower viewing rates and popularity.Öğe High prevalence of high-risk cutaneous squamous cell carcinoma in the thrace region of Turkey(Wolters Kluwer Medknow Publications, 2020) Solak, Sezgi Sarikaya; Yondem, Haydar; Urun, Yildiz Gursel; Cezik, Mert; Can, NurayBackground: The characteristics of cutaneous squamous cell carcinoma (cSCC) may show differences according to the geographic distribution and ethnicity. Although most cSCCs are treated with surgical excision or other local interventions, high-risk cSCCs may have poor outcome. In the present study, we aimed to evaluate the clinicopathological characteristics and determine the high-risk features of cSCCs in the Thrace region of Turkey where the information on cSCCs is scarce. Methods: We retrospectively investigated the biopsy-proven cSCCs diagnosed between the years 2014 and 2018, in a tertiary university hospital and evaluated the high-risk features. Results: A total of 211 cSCCs were included. Men with cSCC were significantly younger than women with cSCC (P <.001). Almost one-half of the tumors (n = 103) were located on the lower lip. Patients with a cSCC on the lower lip were younger than those with a cSCC on the other sites. All patients with a cSCC on the ears were men. Twenty-eight percent of patients had tumor size more than 20 mm and 28% had tumor thickness more than 6 mm. Immunosuppression was present in 7.6% of patients. A total of 177 (83.9%) patients had high-risk cSCC. The local recurrence rate was 6.2%, and the metastasis rate was 5.2%. Conclusions: The Thrace region has significant number of cSCC, and high-risk features are very frequent. Clinicians should carefully evaluate the cSCCs in terms of high-risk features.Öğe Integrated Cadaveric Nail Surgery Course Improves Self-Assessed Knowledge and Competency of Dermatologists(Karger, 2021) Solak, Sezgi Sarikaya; Karakas, Ayse Akman; Sindel, Muzaffer; Atis, Guldehan; Gunes, Pembegul; Goktay, FatihIntroduction: Dermatologists see many patients with nail disorders requiring surgical interventions. However, nail surgery is often found to be difficult by dermatologists. The aim of this study was to evaluate the effectiveness of an integrated nail surgery course. Methods: A nail surgery course model, combining anatomy, pathology, and dermatology with theoretical lectures and hands-on training on fresh-frozen cadaver, was designed for dermatologists. Before and after the course, the participants were asked to complete a questionnaire in which they assessed their knowledge and competence levels. Results: Twenty-eight dermatologists completed the questionnaires. The majority of the dermatologists (79.6%) had limited nail surgery experience, previously. The most commonly cited reasons for lack of knowledge or low or moderate theoretical and/or practical level of knowledge of nail surgery were not having received theoretical and practical education during dermatology residency. The median scores of self-assessed knowledge and competency improved significantly after the course (p < 0.001). The majority (82.1%) of the participants were quite or very well satisfied. Conclusion: An integrative nail surgery course model seems beneficial and encouraging for dermatologists. Organizing this type of nail surgery course, especially in medical schools using fresh-frozen cadavers, may significantly improve dermatologists' knowledge and skills.Öğe Need for patient education and better physician-patient communication in melanoma patients: Behaviors and knowledge of melanoma patients(Deri Zuhrevi Hastaliklar Dernegi, 2021) Solak, Sezgi Sarikaya; Yondem, Haydar; Cicin, IrfanBackground and Design: Melanoma is a serious type of cancer. Patients previously diagnosed with melanoma are at an increased risk of a second melanoma. Therefore, it is important to know and apply preventive measures. This study aimed to examine melanoma patients' behaviors of sun protection and skin self-examination (SSE), knowledge on risk factors, and communication with their physicians about the disease. Materials and Methods: A questionnaire-based cross-sectional study was conducted in 65 melanoma patients to assess knowledge of melanoma and sun protection and determine SSE attitudes and patient-physician communication. Results: Sun exposure was a well-known melanoma risk factor (67.7%), but the knowledge level on other risk factors, especially regarding skin, hair, and eye color phenotypes, was low (between, 21.5% and 36.9%). Sunscreen use practice was insufficient (33.8%) and mostly inadequate. The most commonly reported reason for not using sunscreen was not having a habit of sunscreen use (83.7%). Compliance with other sun protection behaviors was variable. Half (50.8%) of the melanoma patients reported that they perform SSE. The most commonly reported reasons for not performing SSE was not knowing its necessity (71.8%). More than one-third (35%) of patients stated that they did not receive information about melanoma from their physicians. Almost all patients who received information from their physicians were informed verbally, and only 4% received written information. Conclusion: Our study highlights the need for a comprehensive education for patients about all aspects of melanoma and strategies to improve patient-physician communication.
