Açık kalp ameliyatı olan hastalarda solunum egzersizlerinin solunum fonksiyonlarına ve egzersiz toleransına etkisi

Ön test-son test deney ve kontrol grup tasarımlı deneysel planlanan bu araştırma açık
kalp ameliyatı olan hastalarda ameliyat öncesi verilen derin solunum ve öksürme egzersizi
eğitiminin ameliyat sonrası dönemde solunum fonksiyonlarına ve egzersiz toleransına etkisini
belirlemek amacı ile gerçekleştirildi.
Araştırma, 01/10/2019-01/04/2020 tarihleri arasında Edirne ilindeki bir devlet
hastanesinin KVC yoğun bakım ünitesi ve KVC kliniğinde yatan ve açık kalp ameliyatı
geçiren 80 hasta (40 deney grubu, 40 kontrol grubu) alınarak yürütüldü. Araştırmanın verileri
Hasta Bilgi Formu ve Hasta İzlem Formu kullanılarak toplandı.
Deney grubu hastalara açık kalp ameliyatı öncesi en az 24-48 saat öncesinde derin
solunum ve öksürme egzersizleri ile triflow kullanımı öğretildi ve ameliyat zamanına kadar
günde 10-15 defa tekrar etmeleri istendi. Kontrol grubu hastalara ise sadece servisteki rutin
uygulamalar yapıldı. Her iki grupta verilerin toplanması ameliyat öncesi 24-48 saat, ameliyat
sonrası 1. gün ve 3. gün olmak üzere üç aşamada gerçekleştirildi.
Çalışmanın sonucunda derin solunum ve öksürme egzersizleri ile triflow kullanan
deney grubundaki hastaların ameliyat sonrası 3. günde solunum fonksiyon testi değerlerinin
kontrol grubuna göre daha yüksek olduğu, 6 dakikalık yürüme testi yürüme mesafesinin daha
fazla olduğu ve hastaların yürüme hızlarının daha fazla olduğu (p<0,05) belirlendi.
Sonuç olarak; açık kalp ameliyatı olan hastalarda ameliyat öncesi verilen derin
solunum ve öksürme egzersizi eğitiminin ameliyat sonrası dönemde solunum fonksiyonlarını
ve egzersiz toleransını geliştirdiği görüldü. Açık kalp ameliyatı uygulanan kliniklerde çalışan
hemşirelerin derin solunum ve öksürme egzersiz eğitimini ameliyat öncesi dönemde planlayıp
hastalara düzenli olarak uygulatmaları önerilmektedir. This study that had pre-test post-test experimental and control group design and
experimental planning was conducted to determine the impact of deep respiratory and
coughing exercise training given in pre-test period to patients that had open heart operation on
respiratory functions and exercise tolerance of the patients in the post-operation period.
The study was conducted between 01/10/2019 and 01/04/2020 on 80 patients (40
experimental group, 40 control group) who had open heart operation and are at cardiovascular
operation
intensive
care
unit
and
cardio-vascular
operation
clinic
of
a
state
hospital
in
Edirne.
Data
of
the
study
was
collected
using
Patient
Information
Form
and
Patient
FollowUp
Form.
Patients in experimental group were taught deep respiration and coughing exercises
and using triflow at least hours before open heart operation and they were asked to repeat
them 10-15 times until time of operation. Patients in control group were only given routine
applications in the service. In both groups data was collected in three stages being 24-48
hours pre-operation, the first day post-operation and the third day post-operation.
As a result of studies it was determined that patients in experimental group that used
deep breathing and coughing exercises together with triflow had higher respiration function
test values on the third day post-operation, had longer walking distances in 6-minute walking
test and achieved higher walking speeds (p<0.05) compared to the control group.
Consequently, it was noted that deep respiration and cough exercise training given to
patients who had open heart operation pre-operation developed respiration functions and
exercise tolerance in the post-operation period. It is suggested that nurses employed at clinics
where open heart surgery is practiced plan deep respiration and coughing exercise training in
the pre-operation period and have patients apply them regularly.
