Triple negatif meme kanseri olgularında PD-L1 ve Galektin-3'ün tümöre karşı immun yanıt ve prognostik belirteçlerle ilişkisi
Özet
Meme kanseri, kadınlar arasında en sık görülen kanserdir ve kadınlar arasında kansere
bağlı ölümlerin en sık sebebidir. Triple negatif meme kanseri, ER, PR ve HER2/neu
ekspresyonu göstermeyen, genellikle yüksek dereceli, agresif, uzak metastaz riski yüksek ve
hastalık spesifik sağkalımı diğer meme kanseri subtiplerine göre düşük olan bir tümör grubudur.
Çalışmamızda PD-L1 ve Galektin-3’ün triple negatif meme tümörlerindeki ekspresyon
profilinin saptanması amacıyla 62 olgu çalışmaya alınmıştır. İnceleme sırasında tümör alt
tipleri, tümör boyutu, tümör derecesi, lenf nodu metastazı, Ki-67 yüzdesi, TIL yüzdesi
değerlendirilmiştir. Olguların yaş, menopoz, birinci derece akrabada meme kanseri öyküsü
varlığı, rekürrens durumu, ölüm durumu, genel sağkalım ve hastalıksız sağkalım bilgileri
kaydedilmiş, seçilen bloklardan alınan kesitlere PD-L1 ve Galektin-3 antikorları uygulanmış
ve iki gözlemci tarafından ışık mikroskopu ile değerlendirilmiştir.
Elde edilen bulgulara göre PD-L1 pozitifliği ile tümör histolojik alt tipi ve rekürrens
varlığı ile istatistiksel anlamlı fark saptanmıştır. PD-L1 ekspresyonunun hastalıksız sağkalımı
kısalttığı bulunmuştur. Galektin-3 ekspresyonu ile ise rekürrens ve ölüm varlığı arasında
istatistiksel anlamlı fark bulunmuştur. Galektin-3 ekspresyonunun hem hastalıksız sağkalımı
hem de genel sağkalımı kısalttığı saptanmıştır. Ayrıca Gal-3 ekspresyonu arttıkça istatistiksel
anlamlı olarak TIL yüzdesinin düştüğü bulunmuştur. PD-L1 ve Galektin-3 ekspresyonu
arasında ise anlamı fark bulunmamıştır. TIL yüzdesi arttıkça hastalıksız sağkalım ve genel
sağkalımın istatistiksel anlamlı olarak uzadığı saptandı ve bu anlamlılık için risk analizi
yapılarak ölüm ve rekürrens için TIL yüzde kesim değerleri belirlenmiştir.
Sonuç olarak triple negatif meme karsinomu olgularında PD-L1 ve Galektin-3
ekspresyonunun prognostik etkilerinin, hastaların klinik yönetiminde ve mevcut veya yeni
gelişmekte olan immunoterapi ajanlarının endikasyonunun belirlenmesinde faydalı olabileceği
ve aynı zamanda tümörü infiltre eden lenfosit yüzdelerinin hastaların prognostik ön görülerinde
klinik katkı sağlayabileceği düşünülmektedir. Breast cancer is the most common cancer and the most common cause of cancer-related
deaths among women. Triple negative breast cancer is a group of tumors that do not show ER,
PR and HER2 / neu expression and are generally high grade, aggressive with high risk of distant
metastasis in which disease specific survival is lower than other breast cancer subtypes.
In our study, 62 cases were included in the study to determine the expression profile of
PD-L1 and Galektin-3 in triple negative breast tumors. During the study, tumor subtypes, tumor
size, tumor grade, lymph node metastasis, Ki-67 percent, TIL percent were evaluated. Age,
menopause status, breast cancer history in first-degree relatives, recurrence status, death status,
over-all survival and disease-free survival rates were recorded. PD-L1 and Galektin-3
antibodies were applied to the sections taken from the selected tissue blocks under light
microscopy by two observers blinded to the slides.
According to the findings obtained, a statistically significant difference was found
between PD-L1 positivity and histological tumor subtype and tumor recurrence. PD-L1
expression has been found to shorten disease-free survival. A statistically significant difference
was found between Galectin-3 expression and tumor recurrence as well as death. Galectin-3
expression was related with shortened disease-free survival and overall survival. In addition,
the percentage of TIL showed statistically significantly decrease while Galectin-3 expression
has increased. As the percentage of TIL increased, disease-free survival and overall survival
rates were significantly prolonged, and TIL cut-off points for death and recurrence were
determined by performing risk analysis for this significance.
In conclusion, results of our study have shown that PD-L1 and Galectin-3 expression
can be beneficial in clinical management of triple negative breast cancer patients as well as
current or emerging immunotherapy agents. Tumor infiltrating lymphocyte cut-off points may
contribute to the prognostic prediction of triple negative breast carcinoma patients.
