Romatoid artrit hastalarında kinezyofobi sıklığı ve kinezyofobi etyolojisinde santral sensitizasyonun rolü
Özet
Romatoid artrit, özellikle sinovial eklemlerde görülen ağrı, fonksiyonel kısıtlılık ve bu
duruma eşlik eden sosyal ve psikolojik sonuçları olan, kronik inflamatuar bir hastalıktır.
Santral sensitizasyon, santral sinir sistemindeki nosiseptif nöronların normal veya eşik
değerin altındaki afferent uyarılara karşı artmış yanıtlılığıdır. Kinezyofobi, ağrılı yaralanma
ya da tekrarlayan travmalar sonucu aşırı hassasiyet ve rahatsız edici histen kaynaklanan
fiziksel aktiviteye karşı gelişen korkudur. Bu çalışmanın amacı, Romatoid Artrit hastalık
sürecinde gelişen ağrının kinezyofobiye yol açıp açmadığını ve santral sensitizasyon başta
olmak üzere demografik veriler ve hastalık ile ilişkili diğer faktörlerin kinezyofobiye etkisini
göstermektir.
Vaka-kontrol tasarımında yapılan bu çalışma, 72 Romatoid Artrit hastası kadın ve 72
kontrol kadın ile yürütüldü. Katılımcıların sosyodemografik ve klinik özellikleri sorgulandı.
Tüm katılımcıların kinezyofobi durumlarını değerlendirmede Tampa Kinezyofobi Ölçeği,
santral sensitizasyon durumunun değerlendirilmesinde Santral Sensitizasyon Ölçeği, vaka
grubunun ağrı düzeylerini ölçmede Visual Analogue Scale, hastalık aktivitelerini belirlemede
Disease Activity Score 28, depresyon belirtilerini değerlendirmede Beck Depresyon Ölçeği,
günlük yaşam ve kendine bakım aktivitelerini sınıflandırmada Steinbrocker Fonksiyonel
Sınıflama Ölçeği kullanıldı.
Vakalarda kontrollere göre daha fazla kinezyofobi saptandı. Öğrenim düzeyi
yükseldikçe Tampa Kinezyofobi Ölçeği puanları düşmekteyken, beden kitle indeksi arttıkça
artmaktaydı. Romatoid Artrit vakalarında Tampa Kinezyofobi Ölçeği puanları ile Disease
Activity Score 28, Beck Depresyon Ölçeği ve Santral Sensitizasyon Ölçeği puanları arasında
istatistiksel olarak anlamlı şekilde pozitif yönlü korelasyon vardı. Bu bilgiler ışığında,
Romatoid Artrit vakalarının hareketten korkma ve kaçınma tepkilerinin psikososyal, fiziksel
ve algısal faktörlerden etkilenebileceği ve bu tip hastaları kinezyofobi açısından değerlendiren
tedavilerin, asıl hastalığın tedavisi ve rehabilitasyonunun başarısını arttırmada yararlı
olabileceği söylenebilir. Rheumatoid arthritis is a chronic inflammatory disease especially with pain in
synovial joints, functional limitation and social and psychological consequences. Central
sensitization is the increased responsiveness of nociceptive neurons in the central nervous
system to afferent impulses below normal or threshold values. Kinesiophobia is a fear against
to the physical activity, caused by hypersensitivity and disturbing feeling related to painful
injury or repeated trauma. The aim of this study is to evaluate whether the pain developed
during Rheumatoid arthritis disease process causes kinesiophobia, and to show the effect of
the demographics, central sensitization and other factors related to the disease, to the
kinesiophobia.
This case-control study was conducted with 72 women with Rheumatoid arthritis and
with 72 control women. The sociodemographic and clinical features of the participants were
questioned. Tampa Kinesiophobia Scale to assess kinesiophobia and the Central Sensitization
Inventory to evaluate central sensitization of all participants, Visual Analogue Scale was used
to measure patient’s pain levels, Disease Activity Score 28 to determine disease activity, Beck
Depression Inventory to assess depression symptoms, Steinbrocker Functional Classification
Scale to classify daily life and self-care activities.
Cases was found more kinesiophobic compared to the controls. While the level of
education increased, the scores of Tampa Kinesiophobia Scale decreased, and while the body
mass index increased, the scores increased. In Rheumatoid arthritis cases, there was a
statistically significant positive correlation between Tampa Kinesiophobia Scale scores and
Disease Activity Score 28, Beck Depression Inventory and Central Sensitization Inventory
scores. In the light of this information, it can be said that the fear and avoidance responses of
movement of the Rheumatoid arthritis cases may be affected by psychosocial, physical and
perceptual factors and the treatments focused on the kinesiophobia of such patients can be
useful in increasing the success of the treatment and rehabilitation of the disease.
