Hipertansiyon tanısı alan hastalara verilen öz yönetim desteğinin bilgi düzeyi, tedaviye uyum ve öz bakımından yönetimine etkisi
Özet
Araştırma; hipertansiyon hastalarına verilen öz yönetim desteğinin, hipertansiyon bilgi
düzeyine, tedaviye uyuma ve öz bakım yönetimine etkisini belirlemek ve hastaların kan
basınçlarındaki değişimi incelemek amacıyla randomize kontrollü olarak yapıldı.
Araştırma, Temmuz 2017 – Eylül 2018 tarihleri arasında, 137 hasta (69 müdahale, 68
kontrol) ile yürütüldü. Veriler, “Hasta Bilgi Formu”, “Hipertansiyon Bilgi-Düzey Ölçeği”,
“Hill-Bone Hipertansiyon Tedavisine Uyum Ölçeği” ve “Kronik Hastalıklarda Öz-Bakım
Yönetimi Ölçeği” kullanılarak toplandı. İlk görüşmede her iki gruba anket formları ve
müdahale grubundaki hastalara öz yönetim desteği uygulandı, kan basıncı takipleri yapıldı.
Son görüşmede anket formları her iki gruba tekrar uygulandı. Veriler, tanımlayıcı istatistiksel
yöntemlerin yanı sıra Pearson ki-kare testi, Fisher-Freeman-Halton Exact Test, Independent
samples t- test, Wilcoxon testi Paired samples test, Mann Whitney U testi ve Spearman’s
Korelasyon Analizi kullanılarak değerlendirildi. İstatistiksel anlamlılık sınır değeri p<0,05
olarak kabul edildi.
Müdahale ve kontrol grubundaki hastaların tanıtıcı ve hastalığa ilişkin özellikler
açısından benzer olduğu bulundu. Müdahale grubundaki hastalara uygulanan özyönetim
desteği sonrasında, müdahale ve kontrol grubunda yer alan hastaların son görüşmede ölçek
toplam puanları ve kan basıncı değeri ortalamaları arasında istatistiksel olarak anlamlı
farklılık olduğu bulundu (p<0,001). Öz yönetim desteği verilen müdahale grubunda
Hipertansiyon Bilgi-Düzey Ölçeği ve Kronik Hastalıklarda Öz-Bakım Yönetimi Ölçeği
toplam puan ortalamalarının anlamlı olarak yüksek, Hill-Bone Hipertansiyon Tedavisine
Uyum Ölçeği toplam puan ortalamalarının ve kan basıncı değeri ortalamalarının ise anlamlı
olarak düşük olduğu belirlendi (p<0,001).
Sonuç olarak hipertansiyon tanısı koyulan hastalara verilen öz yönetim desteğinin
hastaların hipertansiyon bilgi düzeyini, hipertansiyon tedavisine uyumunu ve öz bakım
yönetimini arttırdığı, ayrıca hastaların kan basıncı değeri ortalamalarını düşürmede etkili
olduğu belirlendi. The research was conducted in a randomized controlled manner to determine the effect
of self-management support given to hypertension patients on the knowledge level of
hypertension, compliance with the treatment and self-care management, and to investigate the
changes in patients’ blood pressure.
The research was conducted between July 2017 - September 2018 with 137 patients (69
intervention and 68 control patients). The data were collected using the “Patient Information
Form”, “Hypertension Knowledge-Level Scale”, “Hill-Bone Compliance to High Blood
Pressure Therapy Scale” and “Self Care Management Process in Chronic Illness”. In the first
interview, questionnaire forms were applied to both groups. Blood pressure monitoring was
performed and self-management support was applied to the patients in the intervention group.
In the final interview, questionnaire forms were re-applied to both groups. In addition to
descriptive statistical methods, the data were evaluated using Pearson’s chi-squared test,
Fisher-Freeman-Halton Exact Test, Independent-samples t-test, Wilcoxon test, Paired samples
test, Mann Whitney U test, and Spearman’s Correlation Analysis. Statistical significance limit
value was considered as p<0,05.
The patients in the intervention and control groups were found to be similar in terms of
descriptive and disease-related characteristics. After self-management support was applied to
the patients in the intervention group, at the final interview, a statistically significant
difference was found between the total scale scores and mean blood pressure values of the
patients in the intervention and control groups (p<0,001). In the intervention group where
self-management support was provided, the total mean score of Hypertension KnowledgeLevel
Scale and Self Care Management Process in Chronic Illness were found to be
significantly high and mean total score of Hill-Bone Compliance to High Blood Pressure
Therapy Scale and mean blood pressure value were found to be significantly low (p<0,001).
As a result, it was determined that self-management support provided to patients
diagnosed with hypertension increased their knowledge of hypertension, compliance to
hypertension treatment and self-care management, it was also found to be effective in
lowering the mean blood pressure values of the patients.
