Kemoterapi alan hastalarda semptom kontrolü ve hemşirelik bakımından memnuniyet
Özet
Kesitsel ve tanımlayıcı tipteki bu araştırmanın amacı, kemoterapi alan hastaların
yaşadıkları semptomların şiddetini ve hemşirelik bakımından memnuniyetlerini belirlemek ve
aralarındaki ilişkiyi incelemektir.
Araştırma; Ocak 2015 – Kasım 2015 tarihleri arasında bir üniversite hastanesinin
onkoloji kliniklerinde kemoterapi tedavisi alan 141 hasta ile yürütüldü. Veriler, “Hasta Bilgi
Formu”, “’Kanserli Hastalarda Edmonton Semptom Tanılama Ölçeği”, ve “Kemoterapi
Hastalarında Hemşirelik Bakımından Memnuniyet Ölçeği” kullanılarak toplandı.
Araştırmaya katılan hastaların yaş ortalamasının 53,35±10,24, %53,9’unun erkek,
%79,4’ünün evli, %64,5’inin ilköğretim ve altı eğitim seviyesinde, %62,4’ünün gelirinin
giderine eşit olduğu, %44’ünün ilçede yaşadığı belirlendi. Hastaların %39’unun klinik
tanısının gastrointestinal sistem kanserleri ile ilgili olduğu, %64,5’inin kanser evresinin ikinci
evrede, %74,5’inin kemoterapi tedavisine başlama süresinin 0-1 yıl arasında ve %30,4’ünün
beş kür ve üzerinde kemoterapi aldığı belirlendi.
Hastaların Edmonton Semptom Tanılama Ölçeği’nden aldıkları toplam puan
ortalamalarına göre en şiddetli yaşadıkları ilk üç semptomun, yorgunluk (6,02±3,8), endişe
(4,97±3,3) ve üzüntü (4,75±3,27), Kemoterapi Hastalarında Hemşirelik Bakımı Memnuniyet
Ölçeği toplam puan ortalamasının 102,91±25,33 olduğu bulundu.
Hastaların Kemoterapi Hastalarında Hemşirelik Bakımından Memnuniyet Ölçeği
toplam puan ortalamalarının, hastaların eğitim durumu, aylık gelir durumu, yaşanılan yer,
klinik tanı, hastalık evresi, kemoterapi kür sayısı, tedavi süresi ve şekline göre istatiksel
olarak anlamlı farklılık gösterdiği belirlendi (p<0,05). Hastaların yaşları arttıkça hemşirelik
bakımından memnuniyetlerinin arttığı, alınan kemoterapi kür sayısı arttıkça hemşirelik
bakımından memnuniyetin azaldığı bulundu (p<0,05).
Hastaların yaşadıkları ağrı, bulantı, üzüntü, endişe, uykusuzluk, iştahsızlık ve kendini
iyi hissetmeme semptomlarının şiddeti ile hemşirelik bakımından memnuniyet düzeyleri
arasında negatif yönde anlamlı bir ilişki olduğu, hastaların yaşadıkları semptomların şiddeti
arttıkça memnuniyet düzeylerinin düştüğü bulundu (p<0,05).
Bu sonuçlar doğrultusunda, hemşirelik bakımından memnuniyeti arttırmak için
öncelikle kemoterapi alan hastaların yaşadıkları semptomların şiddetinin belirlenmesi,
sonuçlar doğrultusunda hastanın bakım ihtiyaçlarının tespit edilmesi ve gerekli bakımın
verilmesi önerilmektedir. The aim of this cross-sectional and descriptive study was to determine the severity of
symptoms experienced by patients receiving chemotherapy and their satisfaction with nursing
care and to examine the relationship between them.
The study was carried out on January 2015 – November 2015 in an oncology service of
a hospital with 141 patients undergoing chemotherapy treatment. The data were collected with
“Patient Information Form”, “Edmonton Symptom Assessment Scale for Cancer Patients”,
and “The Satisfaction with Nursing Care Scale in Chemotherapy Patients”.
The mean age of the patients included in the study was 53.35 ± 10.24 years, 53.9% were
male, 79.4% were married, 64.5% were in primary and lower education level, 62.4% were
equal to income expenses, 44% live in the country were determined. 39% of the patients were
diagnosed with gastrointestinal system cancers, 64.5% were in the second stage of the cancer
stage, 74.5% were between 0-1 years of initiation of chemotherapy and 30.4% received five
cycles or more of chemotherapy.
According to the total score average of the patients obtained from Edmonton Symptom
Diagnosis Scale the first three most severe symptoms were founded fatigue (6.02 ± 3.8),
anxiety (4.97 ± 3.3) and sadness (4.75 ± 3.27). The total score average of the Nursing Care
Satisfaction Scale in Chemotherapy Patients was 102.91 ± 25.33. It was determined that the
total score average of the patients' satisfaction with nursing scale showed statistically
significant difference according to education level, monthly income status, place of residence,
clinical diagnosis, disease stage, number of chemotherapy cures, duration of treatment and
shape (p <0.05). It was found that as the age of the patients increased their satisfaction with
nursing care, and as the number of chemotherapy cures increased, satisfaction with nursing
care decreased (p <0.05). Patients experienced pain, nausea, sadness, anxiety, insomnia, loss
of appetite and self-feeling symptoms with the severity of satisfaction levels of nursing care
was a significant relationship between the negative, while the severity of the symptoms
experienced increased satisfaction levels were decreased. (p<0.05).
In line with these results, in order to increase the satisfaction of nursing care, firstly, it is
recommended to determine the severity of the symptoms experienced by the patients
receiving chemotherapy, to determine the care needs of the patients and to provide the
necessary care.
